   In order to obtain a certificate of completion to commemorate your completion of the course, complete the post-course quiz and submit your answers as an attachment in the form below.   If over 75% of your answers are correct, you will be emailed a Diversity Made Simple Certificate of Completion – which you can use in support of an application for continuing education credit approval.  If less than 75% of your answers are correct, you will have the opportunity to retake the quiz to improve your score.
